Are you a patient in Iwade? We would like your views.
The contract with DMC Healthcare at Iwade Health Centre ends on 31 March 2022. This means we are now considering how GP services will be delivered in the future for patients registered with the practice after this date.
We recognise how important it is to have a local GP practice in Iwade. We are committed to the future of this practice. All options we consider as part of this work will include a long-term replacement with like-for-like services at the health centre.

There are a number of things we must consider when taking decisions on future services. One of these is patient experience and feedback, to help us identify whether there are any areas of general practice that could be developed or improved.
We would be very grateful if you could let us know your views, so we can better understand what is important to you and to help us shape the future service.
